What is Fanuc SP9004 Alarm?

Looking for some better info on this servo fault and some other Fanuc servo alarms.  Any Fanuc technicians have details or a straightforward, simple explanation for the SP9004 alarm?

SSPA:04 POWER SUPPLY ERROR

Check the voltage of the power input to the Power Supply (PS) and the connection status.

The Power Supply (PS) found a missing power supply phase. (Power Supply (PS) alarm E)
